An Ohio insurance agent has had his license revoked over allegations of forgery and impersonating a policyholder.

The Ohio Department of Insurance revoked the license of Frederick Brisker after finding that he submitted a document containing a forged signature to an insurer in support of his own disability claim. The form was purportedly signed by Jack Murphy, an independent regional sales director for Midland Insurance Company, an insurer for which Brisker was a general agent.

The form was sent to “Frederick Brisker & Associates, Attn: Jack Murphy” at Brisker’s business address, according to the Ohio Department of Insurance. Murphy, however, testified that he had never signed the form and was unaware of its existence until the insurer Unum contacted him. Murphy had never worked for Brisker or received mail at Brisker’s office.

Brisker testified that he had no idea how Murphy’s signature ended up on the form, although he admitted to filling out the rest of the form. However, Brisker’s office manager, Carrie Roe, testified that Brisker had referred to the document in her presence as “the form he signed Jack’s name to.”

Brisker admitted to the Ohio Department of Insurance that he had also impersonated a client to the Midland Insurance Company in an attempt to receive information about a policy.
